Two safety alerts issued highlight lack of risk assessment

In the first incident, a worker received serious lacerations to his forearm that required emergency surgery.

In the second incident, a forklift involved in a dual lift operation was tipped over.

Both incidents occurred in late September, and had the following similar contributing factors:

  1. A risk assessment was not completed before commencing the task.

  2. The equipment used, may not have been the most suitable for the task at hand.

A risk assessment would have highlighted the appropriateness of the equipment used.

All workers are reminded they have a duty under the Northern Territory’s work health and safety legislation to take reasonable care of their own health and safety.
