Police are appealing for witnesses following a crash in Port Melbourne on December 11.
A blue Ford Focus was travelling on Citylink and failed to negotiate the left-hand bend to travel onto the Westgate Freeway about 11.15am.
The vehicle has collided into the barriers separating the east and west turn off.
A female occupant of the car was taken to hospital after being located unconscious at the scene of the crash.
A man, who was also travelling in the vehicle, received minor injuries.
