Police are appealing for public assistance to help locate missing man John.
The 77-year-old was last seen leaving Bell Street in Preston on 24 September about 4pm.
Police and family have concerns for his welfare due to medical conditions including dementia.
John is described as having a slim build, grey hair and a beard.
Police have released an image of John in the hope that someone can provide information on his current whereabouts.
Anyone who sights John or has information on his whereabouts is urged to contact Reservoir Police Station on 9460 6744.
