Police and emergency services responded to reports of a two-car collision in Keilor East.
Two vehicles collided on the Old Calder Freeway near the intersection of Keilor Park Drive about 5.15pm.
Investigators have been told a silver Mercedes van east bound crossed the road and collided head on with a black SUV.
A 54-year-old woman who was driving the silver van sustained fatal injuries and sadly died at the scene.
Three people in the black SUV sustained serious injuries and were all taken to hospital.
