Police are appealing for public assistance to help locate missing woman Carol Ann Smith.
The 66-year-old was last seen at a care facility on Swallow Street, Port Melbourne about 12.30pm on 24 November.
Police and family have concerns for her welfare as she has a medical condition.
Investigators have released an image of Carol in the hope someone recognises her and can provide information regarding her current whereabouts.
Carol is described as 151cm tall, thin build with shoulder-length brown hair.
Carol is known to frequent the St Kilda and Windsor areas.
Anyone who sees Carol is urged to contact the South Melbourne Police Station on 9257 3800.
